Insurance Information

 

Central Mass PT is a provider for the following insurances:

  • Aetna

  • Blue Cross Blue Shield

  • Cigna

  • Fallon Select

  • Harvard Pilgrim Health Care

  • Medicare

  • Tufts

  • United Health Care

  • Unicare

  • Workman’s Comp

  • We are also out of network providers for many other insurances. Check with you carrier to find out if we are covered.

Co-payments are collected at time of service unless prior arrangements have been made with the Office Manager. We accept cash, checks, Mastercard, Visa, American Express, Debit Cards, and Medical Flexible Spending Accounts. Health insurance policies are an arrangement between the insurance carrier(s) and the patient. CMPT will prepare insurance forms, and will bill only as a courtesy to the patient. Depending on insurance coverage, patients may be responsible for a portion of their care. Contact your insurance carrier for details.

Fitness Training

Use Your Wellness Benefits!

Some insurance providers have wellness benefits that you can use to cover your fitness training. Call your insurance provider today about your yearly benefits. We are happy to assist you in insurance reimbursement!

Self-Pay Option

We also offer a self pay option for those who have expended their insurance benefits at another facility, have an insurance provider that is not contracted with our facility, have high deductible plans, are uninsured, or are seeking services not billable to their insurance provider (unrelated to an injury or condition- such as improving sports performance).

 

  • PT Initial Evaluation - $125

  • Sport Assessment (Movement Screen) - $125

  • 60-Minute PT Treatment - $100

  • 30-Minute PT Treatment - $75

FAQ's

Billing & Insurance Questions

What is the CMPT cancellation and no-show policy?


If you are unable to attend a scheduled appointment, please notify us as soon as possible. Since we hold a dedicated appointment slot for you, appointments cancelled or re-scheduled within 24 hours will be subject to a $50 cancellation fee.




When are my co-payments due?


Co-payments are due at time of service unless prior arrangements have been made with the Office Manager. Patients are expected to keep payments current.




How much will my insurance cover?


It depends on your insurance plan. Some insurance plans cover 100% or require a co-payment at the time of the visit, while others require that the patient pay a percentage or deductible. Contact your insurance provider for specifics regarding your individual plan, or contact our office for assistance. See our insurance information page for more information.




What if my insurance provider does not cover PT?


We offer a self pay option for those who have expended their insurance benefits at another facility, have an insurance provider that is not contracted with our facility, have high deductible plans, are uninsured, or are seeking services not billable to your insurance provider (unrelated to an injury or condition- such as improving sports performance). $110 for an initial evaluation $110 for a movement assessment $90 for a 60-minute treatment session $65 for a 30-minute treatment session
