Policies

Thank you for choosing Central Mass Physical Therapy and Wellness to evaluate and provide you with your physical therapy needs. Please take a moment prior to your appointment to review our policies.

Billing Policy

  • Currently, CMPT is a provider for BCBS (all plans), Harvard Pilgrim Health Care, Fallon Select, Aetna, Cigna, United Health Care, and Workman's Compensation.
  • If you are not covered by a provider listed above we charge $110 for an evaluation, $90 for a sixty minute visit, and $65 for a thirty minute visit.
  • Co-Payments are to be paid at time of service. We currently accept cash, checks, or credit cards.
  • Deductible and percentage payments will be billed at the time of payment from your insurance company. Payment is due within 30 days of the date of the invoice.
Cancellation Policy
Cancellations must be made at least 24 hours before your scheduled appointment or you will be charged with a $25 fee.
What to Bring
Please come to your first appointment ten minutes early, call our office ahead of time with any questions regarding your insurance referral. For your first appointment you should wear either stretch pants or shorts and sneakers. Make sure to bring your insurance card and a prescription if you were given one.
Directions to our office
Take Rt. 190 to exit 4. Take exit 4 towards West Boylston. Turn right at the end of the ramp toward Greendale. Proceed 0.4 miles to 1078 West Boylston Street. We are located on the left.
