1. What do I need for my first visit?
- Comfortable clothes (athletic shoes and any inserts if applicable)
- Doctor’s prescription for physical therapy
- A lit of medications you take
- Co-payment (cash, credit card, or checks are accepted)
- Insurance Card
2. What do I need to do to assure insurance covers my visit?
- All insurances require a prescription for physical therapy. (This can be brought on your first visit or faxed to us (508-852-3777)
- Currently Blue Cross Blue Shield is the only provider who requires a referral from your primary doctor
- Unicare requires you to contact them at least 24 hours prior to your first visit
- In you have not had therapy on this injury or body part in the last 18 months and have a prescription you are likely covered, but to assure coverage you are encouraged to call your insurance company for confirmation.
- It is your responsibility to understand your benefits for PT.
